HART is a primary Care Health Service provided by Rochdale Health Alliance for the homeless and transient within our borough. The team is led by a Registered Nurse.

This service is for the homeless, vulnerable, and transient community of Rochdale, Heywood and Middleton.

We provide a HART Outreach Service every week.
