Homeless Alliance Response Team

HART is a Primary Care Health Service provided by Rochdale Health Alliance for homeless and transient patients within our borough.

The service is led by a Registered Nurse. This service is for the homeless, vulnerable, and transient communities of Rochdale, Heywood and Middleton.

Following the success of our HART Volunteer Service, we now provide a HART Outreach Service.
