The Primary Care Nurse Service supports residents in the Care Homes across on behalf of 5 HMR Priamry care Networks in the borough.

This proactive service in liaison with other health and social care professionals provides annual health checks for every resident, long term condition management, ensures care plans and advanced care plans are in place.
