Social prescribing enables all primary care staff and local agencies to refer people to a link worker. Link workers give people time and focus on what matters to the person as identified through shared decision-making or personalised care and support planning. They connect people to community groups and agencies for practical and emotional support. They work within multidisciplinary teams and collaborate with local partners to support community groups to be accessible and sustainable and help people start new groups. Social prescribing complements other approaches such as ‘active signposting’. Link workers typically support people on average over 6-12 contacts (including phone calls, meetings and home visits) with a typical caseload of 200-250 people per year, depending on the complexity of people’s needs and the maturity of the social prescribing scheme. And all of this at the same time as being a kind and friendly person who is always checking in on others and making sure they are ok. It is such a pleasure to work with Kavanna. Always ready to help everyone and goes above and beyond her role. Never in a bad mood and nothing too much trouble. Always provides help that goes above and beyond. We would ALL be lost without her here at LGP. She is a great supporter, handles everything that she is tasked with. "Kavanna is an aspiring Practice Manager. Since COVID she has taken on a challenging role running the reception area, a shy person it has been great to see her come out of her shell. She has had some fantastic ideas in the Practice and is a very calming nature in the Practice. From the early days when General Practice was supported by FHSA to the subsequent move to Primary Care Groups, Primary Care Trusts and now ICBs he has represented the profession and ensured that General Practice had a voice and that appropriate funding and support from commissioning was forthcoming. He has undertaken various roles and currently supports the General Medical Council in managing complaints, continues in his role as an appraiser, supports the profession as vice chair of Rochdale and Bury Local Medical Committee, Medical Director of Rochdale Health Alliance and Clinical Director for Heywood PCN which has consistently for many years performed well in contract and service delivery across the borough. For the past 5 years he has represented the PCNs on the Local Care Organisation board and the Local Care Executive only recently having retired from those roles. His leadership at practice level is reflected in his significant increase in patient list size from 3000 when he joined 15years ago to the largest list size now in the borough at over 11500. Alongside these roles he continues to undertaking his support as a trainer for junior doctors. Over the past decade he has led, supported and guided the profession in an admirable way and has been a shinning beacon to the young GPs of of today on how to deliver and support leadership within the borough."     Primary Care Network Team < Back Kate Primary Care Network Team Digital Transformation Lead I have worked in various areas of healthcare for around eight years, these areas include private healthcare sales, domiciliary phlebotomy service management and health care support work for an out of hours district nursing team. I have been employed by Rochdale Health Alliance since February 2021, and my achievements so far include working my way up to service lead for the Homeless Alliance Response Team and supporting numerous successful Covid-19 vaccination sessions which helped over two hundred and fifty homeless and transient individuals receive their vaccines. I also took the service to the 2022 Rochdale Diversity Awards where we were proud to be named finalists for the diversity in health and social care award. My current role is a full-time position as the Digital and Transformation Lead for the Middleton Primary Care Network (PCN) which will require me to support increased access to care for patients, by supporting the adoption and/or optimisation of new technology and other initiatives to improve the care offer as well as enabling PCN staff to work more effectively to support the sustainability of general practice services.

The work they have done also gives young people and people who may not have had the same opportunities to to become Nurses without the need to give us there job and become a full time student.
