We hope you take your time to browse our website where you will find lots of useful information about the surgery and the services we offer. This is the place to keep you informed of the changes, until the new site is live and accessible... We apologise for the delay. Our Story Changes to the Practice – 1st April 2025 I hope this letter finds you well. I am writing to inform you of an important update regarding your GP Practice. As of 1st April 2025, following an NHS Greater Manchester Procurement exercise, Rochdale Health Alliance will be taking over the management and operations of the SMC Great Lever / Lever Chambers Practice. Who are we? Rochdale Health Alliance (RHA) was established in 2016. Our Board is made up of practicing GP Partners, who have between them over 100 years of experience working as GPs, and a Non-Executive Director who is also the Chair of RHA. The Directors, supported by an experienced team provide guidance and insight into the challenges and opportunities in General Practice and help us to shape the services to ensure that they meet the needs of patients. RHA is excited to have you as part of our healthcare family and look forward to providing you with exceptional care and service. Our team is dedicated to ensuring a smooth transition and maintaining the high standards you have come to expect. What Can You Expect: You will continue to receive care from the same dedicated healthcare professionals you trust and there will not be any immediate changes that will affect your care or treatment. You do not need to take any action, and you will still be registered at the practice. Appointments and prescriptions will continue as normal. If you have upcoming appointments or repeat prescriptions, these will not be affected. From 1st April 2025, the Practice name will change to 'The Bolton Family Practice'. As part of the national programme to upgrade and improve GP Practice telephony, April also coincides with the introduction of a new telephone system at the Practice. We are still awaiting a date for the change, but please be assured we will keep you all informed. The new system has lots of benefits in how you can reach us, including a call-back service. Starting 1st April, we will also launch a new website and Facebook page to keep you informed of any updates. We will send you a text when these are live. As your new Practice Manager, I am excited to work with the wonderful team of receptionists, nurses, and GPs here in Bolton. I am available to answer any questions or concerns you may have during this transition.     8f9b8ce9-08b4-40b4-9af9-996d87704bbf < Back Immunisation and Vaccination Training/Updates Multiple Ongoing An interactive e-learning package to support the training of all healthcare professional involved in delivering or advising on flu immunisations Flu immunisation is one of the most effective interventions that can provide to reduce both the risk of death and serious illness from flu and the pressures on health and social care services during the winter. This Flu Immunisation e-learning resource is designed to provide all healthcare practitioners involved in delivering the national flu immunisation programme with the knowledge they need to confidently promote high uptake of flu vaccination and effectively deliver the vaccine programme. FIND OUT MORE Public Health England Increasing vaccine uptake Strategies for addressing barriers in primary careEngland is a world leader in childhood vaccinations, with one of the most comprehensive programmes in the world. PHE data show that, although coverage remains high, children’s vaccine uptake has been slowly decreasing since 2012-13. FIND OUT MORE Training standards and resources The national minimum standards for immunisation recommended that immunisers complete initial Foundation Immunisation Training and complete annual update training thereafter. Face-to-face initial training is suggested in the standards but this has been problematic during the COVID-19 pandemic. The team are waiting for national guidance. FIND OUT MORE E-learning for Health E-learning was developed by e-lfh to be used as a fundamental training course and it covers the topics set out in the national minimum standards. If face-to-face or webinar training is unavailable then this e-learning course can be used; FIND OUT MORE E-learning is to be supported with mentor support and supervision plus immunisation shadowing opportunities to develop practical learning and skills. She is gaining confidence and is always trying to learn and find better ways to do things. And all of this at the same time as being a kind and friendly person who is always checking in on others and making sure they are ok. It is such a pleasure to work with Kavanna. Always ready to help everyone and goes above and beyond her role. Never in a bad mood and nothing too much trouble. Always provides help that goes above and beyond. We would ALL be lost without her here at LGP. She is a great supporter, handles everything that she is tasked with. "Kavanna is an aspiring Practice Manager. Since COVID she has taken on a challenging role running the reception area, a shy person it has been great to see her come out of her shell. She has had some fantastic ideas in the Practice and is a very calming nature in the Practice. Practice Management can be very daunting but with all the challenges thrown at her she has certainly shown she had what it takes.     Runner Up < Back RHA Access Team League of Superheroes Award Runner Up I would like to nominate Sue and her team who support delivery of the Access Service. No matter what trials and tribulations the team face (and there have been many) they step up to deliver enhanced access across 5 PCNs in the borough - 6 days a week / In Hours service 5 days a week and also the winter pressure hubs that ran November to March 2024. All this requires staffing / stock management / appointment book management / IT / communications / dealing with issues and much much more. On many occasions if admin staff have cancelled at such short notice and they are unable to find a replacement for the shift then they cancel their own plans (evenings and weekends) to ensure the service can still be delivered for the booked patients which means that staff members can be working a 12 hour day or 6 days a week. The team are diligent in dealing with referrals and manage significant numbers of referrals on behalf of the service, that would otherwise go back to the patients registered practice and they don't complain when having deal with more when the winter hubs are stood up to support winter pressures,. Over the past 12 months the team have dealt with circa 3949 referrals. The team has coped well when they have been down on staffing and they amazingly maintain a really positive 'can do' attitude in difficult times. Take a bow and well done.
