    Just to explain why The Bolton Family Practice Why not the real website? We are currently in the process of developing our exciting new website, this will hopefully be ready in the next few weeks, until then we will be placing any weekly updates on our RHA website page. The last few weeks has been such a rollercoaster of a journey, as well as it being my first time working in Bolton we have also been preparing for our new telephone system. The timing of the of the new system could have been better but unfortunately this is out of our hands and our new number will go live from 8th May 2025 . Text messages and notifications will be sent out to all patients at every opportunity over the next few weeks and our current provider will help this transition by directing patients to the new number. The new telephone system has some great features and will enable our staff to reach you more efficiently. The Below link will give you more information. The majority of HMR practices do provide a minor surgery service to their patient population however, RHA commissions’ two borough-wide minor surgery clinics, which take referrals from the practices who do not offer the service on-house. All minor surgery services are NHS services.

Social prescribing enables all primary care staff and local agencies to refer people to a link worker. Link workers give people time and focus on what matters to the person as identified through shared decision-making or personalised care and support planning. They connect people to community groups and agencies for practical and emotional support. They work within multidisciplinary teams and collaborate with local partners to support community groups to be accessible and sustainable and help people start new groups. Social prescribing complements other approaches such as ‘active signposting’. Link workers typically support people on average over 6-12 contacts (including phone calls, meetings and home visits) with a typical caseload of 200-250 people per year, depending on the complexity of people’s needs and the maturity of the social prescribing scheme.
