- HART Nurses + Triangle of Care
What is Triangle of Care? There are several complex social and health inequalities often experienced by those who are homeless. Effectively tackling these requires a co-ordinated multi-agency response, to both prevent and tackle the harms associated with being and becoming homeless. Social relationships are essential for a human thriving. Throughout one's life, the warmth and quality of one's social relationships have a profound effect on the development of one's sense of self-worth. They can improve our physical and psychological well-being and buffer against the effects of stress, increased depression, and anxiety. Homelessness can cause isolation through a lack of social and health support. The collaboration of health professionals is extremely important to encourage the inclusion of the homeless population. In Rochdale Heywood and Middleton, we have a large cohort of people who come under the umbrella of homeless. Over the last four months, the HART nurses – (Homeless Alliance Response Team) have linked up with the Hepatitis C nurse team from North Manchester General Hospital, in a project called ‘ Triangle of Care’. “Coming together is the beginning, keeping together is progress, working together is success.” Henry Ford. We aim to visit all hostels within the borough and other places homeless people attend. The HART nurse's role is to discuss general health and wellbeing, ensure people are registered with a GP, support with prescribed medication, gain consent to do a mini health check to include BP, pulse and weight and liaise with their GP. They also refer to other relevant services that may be required such as housing, Turning Point, and mental health. The HART nurses also chat, be a kind friendly face and arrange to see the person again if necessary. The Heptatises C Team The Hepatises C team provide support and comprehensive service, which includes an outreach assessment, anyone testing positive is fast-tracked on to treatment, counselling, education, medical assistance and support throughout the process. Working within the Hepatitis C Hep C) Elimination Programme What is Hepatitis C? Hepatitis C is inflammation of the liver caused by the Hepatitis virus. It can be very serious if not treated leading to cirrhosis and cancer, there are often no symptoms of Hep C at first. The cohort we are caring for can catch Hep C from contact with the blood of an infected person, such as sharing needles, sharing equipment used to prepare and inject drugs, razors, toothbrushes, tattoos, piercings, sharing self-harming tools, nail clippers and sexual practices that lead to exposure to blood. The usual treatment is initially a blood test, called an HCV antibody test to find out if someone is infected with the Hep C virus. The ones with acute hepatitis C virus are generally contagious, so by doing this test it can be treated with antiviral medication -taken for several weeks. There is a 95% cure rate. We have now completed testing in multiple hostels across the Rochdale borough, Petrus Hub and the Reach Out café (ROC), with a mixed level of attendance and several people have been identified requiring further treatment for Hep C. We aim to eliminate Hep C for the clients we deal with across the borough, as no one needs to live with or die from Hepatitis C. A new pilot initiative to check for liver cancer in high-risk communities has been rolled out as part of a major NHS drive to catch more cancers earlier and save lives. This has been set up by NHS England , this has led to the team doing the Liver Fibro scans contacting HART to see if they can link into the ‘Triangle of care’. It is felt that the cohort of clients we are capturing could be in the ‘difficult to reach’ people cohort and therefore, they are offering Fibro scans to at-risk individuals that would not be scanned as part of a routine Hepatitis C virus ODN work, alongside the Hep C team and HART within the Rochdale borough. By incorporating this team alongside the HART and Hep C team we aim to develop a strong rapport with these clients, which will encourage more people who otherwise would be reluctant, to come forward for potentially life-saving scans by performing a quick, non-invasive liver scan. FibroScans are offered to adults from the following high-risk groups (cirrhosis and liver cancer): • With, previously had or at risk of Hepatitis C/Hepatitis B • More than 10 years of excess alcohol consumption (this has now been modified to five years due to the broad scope) • With or suspected Alcohol-Related Liver Disease (ARLD) • With or suspected Non-Alcoholic Fatty Liver Disease (NAFLD) • Other (Including Diabetes and HIV) Seven thousand fibro scans were carried out in the UK from June 2022 to January 2023 identifying over eight hundred and thirty people with cirrhosis or advanced fibrosis, a leading cause of liver cancer, with the majority of those identified referred on to further care. Checks are being offered in the community to adults with high levels of alcohol consumption, a current diagnosis or history of past viral hepatitis, or non-alcoholic liver disease, as these factors increase the risk of developing liver cancer. NHS staff are visiting at-risk communities as part of the Hepatitis C Elimination Programme and the programme is being expanded to include a liver health check involving an on-the-spot fibrosis scan which detects liver damage. Around six thousand one hundred people are diagnosed with liver cancer each year, but the number of cases has doubled over the past decade and is expected to continue to rise. Currently, only one in three liver cancers are diagnosed at an early stage, but this programme will help catch more cancers earlier, giving patients a much better chance of surviving the illness. If caught early, patients have a 70-90% chance of survival for five years or more with treatment. Those who are deemed high risk will be provided with information about their level of risk and, where appropriate, will be referred to their GP. If needed, people will be referred straight into a six-month liver surveillance care programme, where they will be partnered with a peer support worker who will continue to check in, as well as offer guidance and help informed by people who have experienced liver disease themselves. Liver cancer can be hard to detect at an early stage and so these checks, for people who have been identified as higher risk, are an essential part of the NHS’s action to find more people with cancer, and in groups that may otherwise find it hard to access lifesaving tests. Collaboration within secondary and primary care is extremely important. Working together can only provide vital care to the people in the HMR borough, ensuring an inclusive approach can only enhance their take in engaging with health professionals and taking some responsibility for their health and well-being. Also knowing someone cares and is willing to support them can only help in their self-esteem, mental health and maybe a more positive outlook towards the future. - The Apprentice Diaries: Week 6
What I did this week as a Content Creator Apprentice... Introduction This week I have been on visits to Petrus and on a journey with the HART nurses experiencing and stepping into their world it has been a captivating and heartwarming experience helping those in need. I've written a few blogs refreshing our website with new content and informing our audience of our services. Also, we discovered some special news that we've been nominated and finalised for the Diversity Awards! How exciting to see us grow and get recognised for our extraordinary services. Day 1: I started my first day of the week, by writing up my blog 'On my visit with the HART Nurses' on the visit to Redfern house with the HART nurses. I wrote about what the HART nurses do and their aims, I also included what Petrus's aims were and what they offer, breaking down the important information for the audience to have a better understanding of one of Primary Cares Health services that's provided by Rochdale Health Alliance. Day 2: Day two was a productive day for me, I was looking forward to Tuesday as I had arranged to meet Elaine (HART Nurse) at Petrus Hub to capture some new content. I wanted some video content showing what they offer at Petrus. Before taking any footage I had to ask for everyone's permission to be in videos or photos, if they agreed they signed a form, some didn't mind being filmed some preferred not to which wasn't a problem due to almost everyone agreeing. At 9 am Petrus Hub serves breakfast for the homeless so they can sit, eat and socialise. On my visit to Petrus Hot food was served, Hepatitis C testing was available, blood pressure was checked and a clothing rack with clothes that had been donated to them. Day 3: On my third day of the week, I looked at the calendar to see what was planned, so I could create future content for our socials. Due to my recent HART visits, I've decided to venture more into the HART nurse world to expand their reputation and post weekly vlogs about what they do, for example showing that they make free breakfast at 9 am for the homeless. I'm hoping to spread awareness about HART and Petrus to help people who might need to visit Petrus and to show appreciation towards people who work there and genuinely care about others. Day 4: Day four was another productive day, on Thursday Ruth and I discussed marketing strategies and looked into some products that we could give out at events. I finished off putting all my short clips from Petrus's visit into one video introducing Petrus and what they do, I put it up into my calendar to remind me when to post. In the afternoon we went to Peter Loo Medical Centre for a meeting to discuss our plans for the future and to see where we're up to. We received some amazing news that we have been nominated for the Diversity Awards, so I was informed to send off some videos and photos in an email by Friday. Day 5: Wow, what a week on my final day I started my blog about my 6th week as a Content Creator Apprentice, and I published the blog for the 'Triangle of Care' that's related to the HART Nurses. Today is the deadline for sending off the emails for the HART and Primary Care Academy Diversity awards that we were nominated for I got stuck in ensuring that the correct information was sent and what footage was worthy. it's been a fantastic week and I can't wait for more productive days and growth!

What I did this week... Introduction In these past weeks, I've been working on various projects, collaborating with my team, and sharing my experiences with the world. As I continue on this exciting journey, my enthusiasm and dedication shine through. Let's dive into my 7th week and make it another amazing chapter in my content creator apprenticeship! Day 1: Monday was mostly all about organising what I had planned for this week. In the morning I had an effective meeting all discussing and including our ideas. I then went on to post my blog on 'Triangle of Care' on our Instagram story and add it to our highlights, I was mostly organising our Instagram highlights and following our guidelines making sure everything was visually pleasing. Due to everyone having the flu now, I decided to create a Flu Jab post as a reminder for the public to get the flu jab, I then popped it in notion for a date to be posted. Day 2: On day two I was out of the office and at Juice Academy training we covered so much that helped me in my content creator apprenticeship. From learning about reactive content to schedule planning, research and insights, and even campaigns, We did a lot of group work that also increased my teamwork skills. This taught me that there's so much more to look into when being a content creator for a company. Day 3: Day three was a productive day, I had the opportunity to help and advise people to join the Primary Care Academy at the job centre in Rochdale. It's wonderful to see myself and Ruth making a positive impact and supporting others in their career journeys. I was also sharing photos on Twitter and Facebook tagging our current location so the public would come join us. It is a great way to document and make our audience aware of these moments. Day 4: On day four I had the opportunity to visit Milnrow Practice with nurse Zoe and learn about the nurse forums. In the session, there were nurses who specialised in certain areas helping to educate other nurses who attended. It was an insightful experience. I'm going to write a blog about more in this area so keep up to date on our socials and website! Day 5: Finally, on day five, I uploaded a post raising awareness for the flu jab ensuring our audience that it is flu season and if you are someone who gets flu more frequently would advise you to contact your GP and get the flu jab, I posted this on Instagram, twitter and Facebook and I then also finished off your week 7 blog as a content apprentice. I have been super productive and dedicated to my content creation journey. I'm excited to see what the future holds next...

- Celebrating Employment Success with the NHS Work Academy
Pre-employment success! HMR Primary Care Academy, in partnership with Rochdale Training and the NHS, we are celebrating the success of our Sector-Based Work Academy. The course focuses on providing excellent customer service to patients, assisting them with queries or appointments as part of a customer service/administration team within the NHS. We've been helping residents in Rochdale gain skills and secure full-time employment in the NHS. In collaboration with Rochdale Training and Job Centre Plus, the course has supported 30 residents, with 13 successfully moving into full-time employment. It's great to see organizations working together to support the community! “ We choose to run Sector-based Work Academies as it’s a great way to attract new staff to Primary Care and a rewarding career in the NHS. The calibre of learners demonstrates that our selection process for the training is effective. How my 10th week went as a content creator apprentice... Welcome back! Today, I want to take you on a journey through my 10th week as a content creator apprentice. It's been a whirlwind of creativity, learning, and exciting experiences, let's dive right in. On the first day of my 10th week, I received exciting approval from Petrus to post my video. It was such a great feeling to finally share my hard work with the world! I also published my pre-employment blog and made sure to promote it on our social media platforms. It's important to get the word out, right? But that wasn't all! I had the opportunity to work on creating a post to announce our joy in January campaign. It was a fun and creative project that allowed me to showcase our company's upcoming plans. I'm looking forward to spreading joy in January! Day 2 was filled with exciting tasks, I dedicated my time to working on the "Joy in January" campaign, exploring various marketing strategies to make it a success. Additionally, I took a significant step by starting MY 10th-week blog, which will allow me to share my experiences and insights with my audience. Looking ahead, I began planning my Wellness Wednesday post for next week, showing great foresight and organization. Day 3 was eventful, In the morning, I had a meeting with Ruth and the Juice Academy, where I likely discussed getting signed on the course. Then I started creating a feedback template and adding it to my calendar. after doing this I started signing up for Bud and completing the assessment. Lastly, after getting Petru's approval I began to post the HART video on TikTok as it was an excellent way to showcase my creativity and engage with our audience. Day 4 was exciting, I kicked off the day by making the Joy Jan announcement, sharing the excitement of the upcoming campaign with our audience. and also keeping the momentum going by continuing to work on my blog, providing valuable content to our readers. moving forward to posting a story about the new pre-employment program in Bury on our social media platforms and website showing our dedication to sharing important updates and opportunities. It was fun to help decorate the office for Christmas, adding a festive touch to the workspace. Of course, lastly, I arranged a meeting with Georgia and Jack to discuss future marketing ideas which was full of creativity and collaboration. Day 5 was a productive one! I accomplished a lot by finishing up my blog, which is a great milestone. I took the time to explore more ideas for the "Joy in Jan" campaign. coming up with some fantastic concepts to spread joy and positivity. lastly, I started to look at Bud and worked on my Juice Academy tasks catching up on work and showing continuous learning and growth.

How my 11th week went... Welcome back to my blog, this is my 11th week as a content creator apprentice at Rochdale Health Alliance. This week was filled with growth, engaging and exciting activities. From working on my Joy in January template to finally announcing it on our socials, I'm enjoying seeing my progress and making my ideas come to life. Day1 Let's dive into day one of my productive day. From editing Peterloo clips to having weekly meetings with Jack and Ruth to discuss marketing plans, my day was filled with creative and productive moments. I also worked on the Joy in Jan template and completed the Wellbeing Wednesday template, showing my dedication to spreading positivity and wellness. And let's not forget about the upcoming opportunity that Better for Business has offered me through Juice Aacdemy as a camera operator in January! I'm super pleased and excited to be making great strides in my career. Day2 Day 2, In my 11th week as a content creator apprentice, I headed to the Juice Academy to expand my knowledge even further. Learning about copywriting and exploring the world of social media platforms. It's great to be dedicated and keep honing my skills and staying up-to-date with the latest trends. I'm excited to learn more at the Juice Academy and how it will influence my content creation journey. Day3 Day 3 was a busy and productive one. displaying our Wellness Wednesday post on our social media platforms which was based on well-being in the workplace and how it is essential, this was a great way to inspire and engage our audience. And later on, having a meeting with Dawn was a significant opportunity to collaborate and present to Dawn my new ideas. AS January is coming up fast, I posted the announcement about our Joy in Jan campaign launching in January, I'm excited to see how it goes and the positive effect it will have on our audience. Lastly, Christmas is coming to an end so is the elf on the shelf, we incorporated the elf-on-the-shelf concept into healthcare and local GP practices from our well-being boxes we gave out to our staff within GP practices, I posted some of 'H'Elf on shelf' version on Instagram and Twitter. It's wonderful to see everyone's cooperation and creativity shining through. Day4 Day 4 was packed with exciting moments. Starting by posting my pre-employment feedback post which was a valuable way to share experiences and insights with our audience. And meeting at Peterloo to discuss our progress and plans shows how invested I am in my work. I'm thrilled to hear about the ideas and plans that were discussed during the meeting. I feel like I'm making great strides in my apprenticeship and leaving a positive impact. Day5 Day 5 was a whirlwind of creativity and productivity, Editing Peterloo videos was a fun and engaging task. Posting a poll about the Elf on the Shelf competition on our social media platforms and website; was a fantastic way to engage our audience and generate excitement. Moving on to finally finishing my Week 11 blog, sharing my amazing insights and experiences. Once I edited the Peterloo videos I posted one on TikTok and Instagram reels these short clips will be released as like a series of workers within primary Care, so keep up to date with us on our social media. Once they were all done I started editing my gratitude journal preparing for the Joy in Jan campaign.

What I've been up to in the last couple of weeks... Hey everyone! I hope you all had an amazing Christmas and New Year's celebration. As for me, I've been back at work, diving headfirst into some exciting projects that I can't wait to share with you all. One of the highlights of the past few weeks was arranging a smear video for educational purposes. It's been a fantastic opportunity to shed light on this important topic and show the public what it's really like. I believe that education is key, and I'm thrilled to be a part of this initiative. In addition to that, I've been working on various other projects that have allowed me to explore my creativity. I ordered gratitude journals and embarked on a "Joy for Jan" campaign, spreading positivity and encouraging others to embrace gratitude in their daily lives. Taking photos of the journals and sharing them on social media has been a great way to promote them and connect with others who are on a similar journey. But my work doesn't stop there! I've also been putting the finishing touches on an abuse campaign leaflet, aiming to raise awareness and provide support to those who need it. It's been a challenging but rewarding experience, knowing that I'm making a difference in people's lives. On a lighter note, I recently visited the local fire station and had a blast filming for an upcoming project. I'm on the lookout for cheap family layouts this January, and I can't wait to share my recommendations with all of you. After all, who doesn't love a fun-filled day with loved ones? Last but not least, I tackled the important topic of smear tests by filming a Q&A video. I wanted to address common questions and concerns, making this vital topic more accessible and less intimidating. It's all about spreading awareness and empowering others to take charge of their health. I'm truly grateful for the opportunities I've had as a content creator apprentice. From exploring the health benefits of plants to promoting diversity in the workplace, every project has allowed me to grow both personally and professionally.
