HMR Primary Care Academy is the vision of Dr Gill Tonge, and is commissioned by NHS Heywood, Middleton and Rochdale Clinical Commissioning Group. The Academy is in innovative solution to addressing the ongoing difficulty in recruiting, developing and retaining GPs, Practice Nurses and Primary Care Staff in HMR.

The Board < Back Dr M Jiva MBE - GP - Director The Board GP Director - Chief Executive Officer Dr Jiva MBE, is the Executive Chief Officer of RHA, a GP at Peterloo Medical Centre Middleton and Chief Executive for Rochdale & Bury Local Medical Committee. Married with two children, Dr Jiva enjoys movies, especially Marvel/DC, eating out and travelling. Dr Jiva is passionate about ensuring that despite the current challenges the NHS faces, the residents of the Rochdale borough receive the best possible healthcare service, by maximising the resources available to us.

Primary Care Academy Team < Back Dawn O'Brien Primary Care Academy Team PCA Manager I am the Manager of Heywood, Middleton and Rochdale Primary Care Academy. I started my career in the NHS 16 years ago working as a receptionist for a GP surgery in Hyde. After 5 years I then moved to an administrator role in the NHS working with a children’s team. After 7 years I then moved to Tameside Hospital as part of the Foundation Team working with junior doctors and their education and I was then offered a post setting up the new Enhanced Training Programme which was to look at the introduction of new roles within Primary Care and developing placements for undergraduate students with a view to encouraging them to consider a career in Primary Care. In July 2018 I was offered the opportunity to become the Manager of the HMR Primary Care Academy working with the wonderful Dr Gill Tonge and Rochdale Health Alliance, developing a team that will hopefully help change Primary Care across HMR! I have a passion for education and feel that with the team we are building then the Academy will go from strength to strength. I am also mum to two wonderful children (most of the time), myself and my partner (Andy) are also foster parents to two other children and have fostered for 7 years, we recently added to the chaos with our adorable Morkie puppy Maggie. We enjoy walking and looking for dog-friendly places to take Maggie Moo to explore!

Primary Care Nurse < Back Primary Care Nurse The Primary Care Nurse Service supports residents in the Care Homes across on behalf of 5 HMR Priamry care Networks in the borough. This proactive service in liaison with other health and social care professionals provides annual health checks for every resident, long term condition management, ensures care plans and advanced care plans are in place.

Access Service < Back Access Service In Hours - Extended Primary Care Hub - Whitehall Street and the Phoenix Centre RHA is commissioned to provide an In Hours Same Day GP Appointments Service. GP Practices are under extreme pressures and appointments can be difficult to get due to this. To try and ensure that patients who, following clinical triage, by their GP practice need to be seen by a GP, can be booked into one of these appointments so that they are seen on the same day. This service runs Monday to Friday between 10:00 and 18:00 at Whitehall Street and the Phoenix Centre. Enhanced Access RHA is commissioned by 3 Primary Care Networks (Heywood, Rochdale North, and the Bridge PCNs) to provide GP and other healthcare appointments outside of Practices core Hours. These appointments are provided between Heywood & The Bridge PCNs - Monday to Friday - 18:30pm and 20:00pm and on Saturdays between 9am and 17:00pm – Heywood PCN appointments are provided at the Phoenix Centre and the Bridge PCN appointments are provided from Nye Bevan House Rochdale North PCN - Rochdale North PCN Monday to Friday18:30-21:30 and on Saturdays between 09:00 – 18:00 and appointments are provided form Whitehall Street. These appointments are routine appointments and are pre bookable by contacting your GP surgery . This can be a mix of GP appointments and Nurse or HCA or Nurse Associate or Phlebotomy appointments. These appointments provide better access for patients who may find it difficult to attend for appointments during the working day for example. Sunday and Bank Holiday RHA is commissioned to provide a service on Sundays and bank Holidays to provide GP appointments on these days between 10:00 and 16:00 on Sundays and 10:00 and 18:00 on Bank holidays. This service is a boroughwide service and these appointments are available to patients of ALL practices in HMR. These appointments are routine appointments and are pre bookable by contacting your GP surgery . In addition the service does take referrals from the Urgent Treatment Centre and NHS 111 where appropriate. Primary Care Academy < Back Primary Care Academy The Primary Care Academy is provided by Rochdale Health Alliance and works in collaboration with NHS Heywood, Middleton and Rochdale Clinical Commissioning Group. The Primary Care Academy was launched in September 2018 to focus on training, education, workforce development and research across the entire primary care workforce in HMR. It aims to create and provide new opportunities for people working in primary care and entering primary care, to shape, inform and influence the career opportunities and environment they would like to work in. We will collaborate with, NHS Heywood, Middleton and Rochdale Clinical Commissioning Group, Higher Education Institutes and other identified stakeholders in supporting service redesign, education and training of the workforce across primary care. The HMR Primary Care Academy is supporting the implementation of the NHS strategic direction, the Five Year Forward View and the programmes that underpin it, particularly the GP workforce 10 point plan, which stresses the urgent need to recruit doctors and nurses into general practice, retain current staff and support returners. Our Key priorities are: To help future proof General Practice in HMR – providing clinical and non-clinical staff with the necessary workforce skills, values and support tools, such as mentoring to improve the quality and continuity of safe primary care services in HMR To promote HMR as the first-choice career location – influence GPs, Nurses and other health professionals planning their future and highlight the many benefits of living and working in the area with an emphasis on introducing roles emerging and apprenticeships available Valuing Primary Care Staff – their role is essential in securing the sustainability of the NHS and acknowledges the need to nurture and support career development. Recruitment and retention- initiatives to support GP Practices Grow our own staff – working with local training providers, schools, colleges and universities Support practices and system leaders – respond to the changing needs of local health and care systems and helps to avoid future vulnerability by understanding and planning for future change now Research and development – to identify those members who would like to be involved in research and form links with universities/networks. HART (Homeless Alliance Response Team) < Back HART (Homeless Alliance Response Team) HART is a primary Care Health Service provided by Rochdale Health Alliance for the homeless and transient within our borough. The team is led by a Registered Nurse. This service is for the homeless, vulnerable, and transient community of Rochdale, Heywood and Middleton. We provide a HART Outreach Service every week.
